FORM 1 (Entering High School for first time)
Requirements
Application fee of $5.00

Official transcript from school (Standard 4, 5 and 6) – No photocopies of report cards
Principal’s Recommendation in sealed envelope (Download Recommendation)
Standard Six Teacher's Recommendation in a sealed envelope (Download Recommendation)
A photocopy of your child's NEW birth certificate
A photocopy of of your CHILD'S social security card
A photocopy of your MOTHER'S social security card
A personal statement from the child explaining in detail why he/she wants to attend Cornerstone Presbyterian High School. (It should be a paragraph of at least six sentences.)
Once all of the above requirements have been met, deliver all documents to the Administrative Office at Cornerstone Presbyterian High School.
An official letter of acceptance will be sent.
FORM 1 TO 4 (Transferring: Non-Repeating or Repeating)

Official transcript from last high school attended
High Principal’s Recommendation in sealed envelope (Download Recommendation)
High School Teacher's Recommendation in a sealed envelope (Download Recommendation)
A photocopy of your child's NEW birth certificate
A photocopy of of your CHILD'S social security card
A photocopy of your MOTHER'S social security card
A photocopy of child's PSE report
A personal statement from the child explaining in detail why he/she is transferring from the previous high school and why he/she wants to attend Cornerstone Presbyterian High School. (It should be TWO paragraphs.)
Once all of the above requirements have been met, deliver all documents to the Administrative Office at Cornerstone Presbyterian High School.
An official letter of acceptance will be sent out.
